  • Appointment of a personal representative (executor)

  • Affidavit of execution (Self-Proving Affidavit) included, if applicable

Living Will / Advance Health Care Directive

A Living Will (also known as an Advance Directive) allows you to state your preferences for medical care if you become seriously ill or incapacitated and unable to communicate your wishes

  • Preparation of a legally compliant Living Will
  • Clear instructions for end-of-life care and life-sustaining treatments
  • Appointment of a health care proxy or agent (optional)
  • Instructions for signing, witnessing, and sharing with healthcare providers
